Athletic Forms
Required before practice/tryouts:
1. Electronic Forms - We are very excited to announce that Zionsville Middle School has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. Starting this year, you may also upload your Physical Form directly to your FinalForms account.

The Parent Playbook (view here) will help get you started in FinalForms. Since FinalForms will now take the place of our previous software (RankOne), ALL parents of athletes must use FinalForms.

2. Physical Form- Must be on the IHSAA physical form and uploaded on Final Forms. In order to be valid for the 2023-24 school year, physical examination must be completed on or after April 1, 2023.
2022-23 Physical Form
Required before first contest: Program Support/Transportation Fees are $50. Fees can be paid online (School Pay) or via check (made out to ZCS). If a student is on free/reduced lunch then the fee acts as such (free/$25). The fee must be paid prior to the first official competition.

ImPACT Testing:
Baseline ImPACT Testing is no longer required for student-athletes. It is NOT necessary to schedule an individual appointment to complete the ImPACT test.
